CCO Oregon creates the space for action-oriented discourse that leads to strategies and solutions that work toward lower costs, better care, improved outcomes, a sustainable workforce, and meeting social needs across the health care delivery system.
Our programs are designed to be nimble and address changing needs. Coordinated care in Oregon is often referred to as the hub and spokes model — at CCO Oregon, we convene the hubs and the spokes.
Our members include the health care payers and providers working across coordinated care. They are coordinated care subject matter experts and innovators who think ahead, explore cutting-edge solutions, and drive systems change across Oregon. They come together for meaningful conversations that spark ideas and the plans to see them to fruition. By acting together, we help them amplify their voices.
We are a statewide, nonprofit 501(c)(3) member association supporting the coordinated care model in Oregon since it was formalized in 2012.
CCO Oregon’s vision is lower costs, increased access to quality care and health-related social needs, improved outcomes, and a sustainable workforce across the delivery system through the widespread implementation of the coordinated care model.
CCO Oregon convenes payers, providers, and other care delivery partners to identify and advance evidence-based, stakeholder-driven strategies that support coordinated care and health transformation.
