In 2022, Oregon celebrated the 10th anniversary of the coordinated care model. Learn more about the model and what it means for providers, coordinated care organizations (CCOs), and Oregonians in communities across the state by watching the video below and please join us for the upcoming 2023 CCO Oregon annual conference this fall.

CCO Oregon creates space for health care payers and providers to engage in action-oriented discourse that leads to strategies and solutions to lower costs, provide better care, improve outcomes, sustain a high quality workforce, and meet patient social needs.

With Oregon’s Coordinated Care Organizations (CCOs) at the core of our work, our membership also includes other health care payers and providers, hospitals and clinic systems, behavioral and oral health organizations, data and analysis partners, and social service agencies. Our members come together for meaningful conversations that spark ideas and the plans to see them to fruition. By acting together, we help them amplify their voices.

CCO Oregon is a statewide, nonprofit 501(c)(3) member association supporting partners across the coordinated care model since it was formalized in 2012.

Vision

CCO Oregon’s vision is lower costs, increased access to quality care and health-related social needs, improved outcomes, and a sustainable workforce across the delivery system through the widespread implementation of the coordinated care model.

Mission

CCO Oregon convenes payers, providers, and other care delivery partners to identify and advance evidence-based, stakeholder-driven strategies that support coordinated care and health transformation.
