The Social Determinants of Health workgroup focuses on efforts to deliver equitable and accessible health services as well as how coordinated care may mitigate the impact of social determinants.
Over the past year, the group has spent time discussing screening, coding, and how electronic health records may incorporate population health and social determinants data, and how multiple provider types may collect, access, and understand this data; how clinical integration models may inform social service integration; opportunities in expanded public health partnerships; and, current efforts in this realm across coordinated care and health systems.
Recent workgroup products include:
- February 2019 comment to the Oregon Health Authority’s Metrics and Scoring and Health Plan Quality Metrics Committees on the use of z-codes for screening and population health analysis
- August 2018 comment to the Oregon Health Policy Board on the CCO 2.0 policy options
- March 2018 comment to the Medicaid Advisory Committee on the definition of social determinants of health
