CCO Oregon hosted a conference focused on the Social Determinants of Health in June 2017 with nearly 300 coordinated care stakeholders in attendance. The workgroup reconvened in November 2017 engaging additional stakeholders from across Oregon representing CCOs, service organizations, federally qualified health centers (FQHCs), insurers, and providers.
In 2018, the workgroup will present public comment at the Medicaid Advisory Committee and other governing bodies regarding the potential of coordinated care to mitigate the adverse effects of social determinants of health on populations served. Targeted workgroup projects may examine: housing risk, kindergarten readiness, food insecurity, and more.
